All testing must be done through a physician. Take the mailing materials and all information
to your doctor's appointment.
When your physician orders the test you will have your blood drawn using the blood tube provided in the kit. The
blood will need to be shipped the same day for next morning delivery.
*Note specimen must be received within 48 hours of being drawn.
Results will be faxed to the ordering physician 4-5 days after receiving the specimen. All results must be
obtained through the physician; our laboratory can not give out results except to the physician.
Patients must pay for testing at the time of specimen submission unless the hospital,
laboratory, or physician office is willing to handle the billing. WVRG is unable to bill insurance companies.
A personal check, money order, or MasterCard, VISA, Discover and American Express are acceptable methods of
payment.
Cost of testing is listed on the enclosed "Diagnostic Testing Services" sheet.
*Note the $34.00 mailer cost should be included with payment.
WVRG is not a Medicare provider. Therefore Medicare will not be billed for our laboratory testing.
After testing is completed you will be sent a receipt for the tests you paid for along with a
Health Insurance Claim Form. The claim form will have the general laboratory and testing codes information
filled in. The information that is specific to your case will need to be filled in by you.
1) Date of service (found on receipt from lab)
2) Diagnosis codes (ICD9 codes from physician)
*Note you may want to get the ICD9 code when you see your physician to have your blood drawn
